This form is designed for U.S. lawful permanent residents (green card holders) whose permanent resident card (green card) is lost, stolen, damaged, is expiring, or contains incorrect information. In order to update (change your name), renew or replace a permanent resident card, a U.S. lawful permanent resident must complete and file Form I-90. This form should not be used by conditional permanent residents who wish to remove conditions on their residence.
